POSITION DESCRIPTION

This position is responsible for providing pharmaceutical care and education to the patients of the Fallon Tribal Health Center and supports all related activities to ensure that high-quality care is provided to all patients.

ESSENTIAL DUTIES & RESPONSIBILITIES

1. Involved in defining and communicating the Pharmacy mission, vision and values to the Pharmacy Department.
2. Ensures outpatient care services are based on identified patient needs.
3. Ensures the Pharmacy staff demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients.
4. Ensures staff members require knowledge of age specific medication requirements for all age groups; each age group requires accurate knowledge of drugs and their appropriate use.
5. Formulates Pharmacy policies and procedures, interprets pharmacy administrative policies, directives and procedures to employees and patients.
6. Implements, coordinates and supervises pharmacy activities.
7. Attends and participates in facility and service unit committees.
8. Establishes and monitors the pharmacy program through development, implementation and review/revision to ensure compliance with accrediting bodies, this is to include, prescription dispensing, drug distribution, patient consultation, drug therapy monitoring, pharmacy primary care (acute and chronic), and pharmacy quality assurance.
9. Provides direct pharmaceutical services to patients.
10. Selects, procures, prepares and dispenses only those medications that conform to standards of quality for potency, purity and effectiveness at the time they are administered to or used by patients.
11. Maintains the necessary legal records on controlled substance drugs and other prescription products.
12. Reads and interprets prescriptions, computer dosages; weighs and measures drugs and mixes and compounds drugs and chemicals.
13. Provides specialized expertise in the proper dosing of dangerous agents know to have narrow therapeutic benefit to toxic ratios.
14. Resolves problems related to the clinical, chemical and therapeutic applications of drugs, including biopharmaceutical effectiveness, utilization of new therapeutic agents and special formula and dosage requirements.
15. Reviews medical records for safety, therapeutic appropriateness, diagnosis/treatment and effectiveness of drug therapy; patient allergy and medication compatibility with other current medication (drug interaction screening) and notifies provider/prescriber concerning any discrepancies.
16. Consults with physicians on the significance of properties of drugs regarding absorption, metabolism and excretion in all age groups.
17. Evaluates all prescriptions for appropriateness to the age, weight and gender of patient; every patient age 12 and under will have a profile which contains the weight and age.
18. Discusses the role of medicines with patient and/or family members, in patients aged 12 and under, this includes all adult patient representatives present.
19. Consults with medical staff on other choices of medicine if deemed inappropriate for the age group.
20. Collaborates with nursing to evaluate medication use in all age groups.
21. Patients 65 and older will be monitored for renal functions and subsequent dosage adjustments. Pharmacist must ensure staff have the knowledge of principles of growth and development over the span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age-specific needs and to provide the care needed.

KNOWLEDGE, SKILLS & ABILITIES

1. A Bachelors, Masters or Doctoral degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).
2. Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy, or
3. Documentation of appropriate education, training and/or CME in the practice of clinical pharmacy
4. Three (3) to five (5) years’ experience as a Pharmacist

QUALIFICATION REQUIREMENTS

1. A current, valid, unrestricted license to practice Pharmacy in the United States, District of Columbia, Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands. Must be in good standing!
2. Board certification in one or more of the pharmacy specialties recognized by the Board of Pharmaceutical Specialties.
